Exercise intolerance is the cardinal symptom of heart failure (HF) and is of crucial relevance, because it is associated with a poor quality of life and increased mortality. While impaired cardiac reserve is considered to be central in HF, reduced exercise and functional capacity are the result of key patient characteristics and multisystem dysfunction, including aging, impaired pulmonary reserve, as well as peripheral and respiratory skeletal muscle dysfunction. We herein review the different modalities to quantify exercise intolerance, the pathophysiology of HF, and comorbid conditions as they lead to reductions in exercise and functional capacity, highlighting the fact that distinct causes may coexist and variably contribute to exercise intolerance in patients with HF.  相似文献

The diagnosis of exercise-induced laryngeal obstruction (EILO) should be suspected when exercise triggers inspiratory stridor. EILO is common in adolescent populations and has a significant impact on sports. Identification of laryngeal obstruction during continuous laryngoscopy during exercise (CLE) is the gold standard diagnostic tool for this disorder, which is not widely known in France. The challenge faced by otolaryngologists is to identify, among patients referred by pulmonologists or sports physicians, those with exercise-induced inspiratory symptoms or poorly controlled exercise-induced asthma, in whom a diagnosis of EILO is strongly suspected. Laryngoscopy at rest may reveal a laryngeal, glottic or supraglottic abnormality predictive of obstruction at increased inspiratory airflow. When pulmonary function tests are normal or in the case of failure of treatment of exercise-induced asthma, the otolaryngologist must complete the examination by a CLE test to confirm the diagnosis of EILO and identify the site of obstruction. This examination is well tolerated, minimally invasive and allows identification of the site of airflow obstruction, allowing specific conservative or surgical treatment. This technical note describes in detail clinical examination and CLE testing in patients with suspected EILO.  相似文献

Evidence suggests that yoga can improve executive functioning (EF) and psychological well-being, but there is no evidence whether flow-based, moderate-intensity forms of yoga can deliver similar benefits. The purpose of this randomized controlled trial was to test the aforementioned effects of engaging in a 30-min, video-guided sun salutation exercise (n = 25), relative to aerobic exercise (n = 22) and attentional control (n = 24). Low-active participants (Mage = 27.84; 87.3% female), with symptoms of stress, completed psychosocial assessments and tasks assessing EF, before and after the session. Results showed a significant group difference for state anxiety (p = .01, ηp2 = 0.13) and stress (p < .05, ηp2 = 0.09), in favor of the yoga group. No significant between-groups differences in EF were observed. Results suggest that video-guided, flow-based forms of yoga can induce immediate improvements in well-being, yet extended involvement may be required to realize performance improvements in memory and processing speed.  相似文献

目的:比较两种家庭训练方案对腰痛患者的临床疗效。方法:门诊招募60例腰痛患者随机分为常规组(n=30)和核心组(n=30)。两组患者均接受腰痛防护知识宣教及评估。常规组接受常规家庭训练方案指导;核心组接受麦吉尔(McGill)"三大训练"(Big3)家庭方案指导。之后两组患者独立完成6周家庭训练。训练前后采用汉化Oswestry功能障碍指数(Oswestry disability index,ODI)和疼痛目测类比定级法(visual analogus scale,VAS)作为主要评估指标,腰部活动度和躯干肌群的肌耐力作为次要评估指标对患者治疗效果进行评估。根据患者训练的完成度、自觉帮助度及满意度评估患者治疗依从性及家庭训练方案的可行性。结果:常规组和核心组分别有24例和25例完成了家庭训练。经过6周家庭训练后,两组患者的ODI及VAS均有明显降低(P<0.01)、腰部ROM均有明显增加(P<0.01);常规组部分躯干肌群和核心组全部躯干肌群的肌耐力明显增强(P<0.01);核心组的ODI、VAS和右侧侧方肌的肌耐力与常规组比较差异具有显著性意义(P<0.05);核心组患者的自觉帮助度及满意度得分与常规组比较差异具有显著性意义(P<0.01)。结论:两种腰痛家庭训练方案均可行并有效。与常规家庭训练方案相比,Big3家庭训练方案对于改善腰痛患者疼痛和功能障碍方面具有更好的临床疗效,并且患者的接受度更高。  相似文献

目的 系统整合脑卒中幸存者运动依从性的影响因素。方法 计算机检索PubMed、Web of Science、Cochrane library、Embase、CINAHL、PsycINFO、百度、Google、中国期刊全文数据库、万方数据库、中国生物医学数据库和维普中文期刊数据库中有关脑卒中运动依从性影响因素的相关研究,检索时间自建库至2021年11月15日。2名研究员独立筛选、提取、评价文献,遇到分歧小组协商讨论。结果 纳入符合标准的文献13篇,共提炼出完整的研究主题62个,归纳组合形成6个新的类别,2个综合的整合结果。整合结果1: 影响脑卒中患者运动依从性的个人因素;包括个人动机因素,个人能力因素及个人心理因素;整合结果2:影响脑卒中患者运动依从性的外在因素;包括人际关系因素、康复团队因素及社会系统因素。结论 深入理解影响卒中患者康复运动依从性的驱动和阻碍因素,找准提高康复运动依从性的突破口,进而提高患者的运动积极性,提高其生活质量。  相似文献

Molecules such as cytokines, energetic substrates, and hormones found in the immune cell environment, especially lymphocytes and monocytes, are crucial for directing energy metabolism. In turn, changes in energy metabolism occur in a synchronized manner with the activation of certain signaling pathways, thereby this crosstalk is responsible for determining the functionality of immune cells. The immunometabolism field has grown over time and that is becoming increasingly promising in several populations; here we discuss the mechanisms involved in sedentary and physically active middle-aged individuals and master athletes. In this context, this review shows that the physical activity status and lifelong exercise seems to be good strategies for the promotion of metabolic and functional adaptations in T lymphocytes and monocytes, counteracting inflammatory environments caused by expanded adipose tissue and sedentary behavior, as well as delaying the immunosenescence caused by aging.  相似文献

ObjectivesWe investigated psycho-physiological responses to perceptually regulated interval walks in hypoxia versus normoxia in obese individuals.DesignWithin-participants repeated measures.MethodsTen obese adults (BMI = 32 ± 3 kg/m?2) completed a 60-min interval session (15 × 2 min walking at a rating of perceived exertion of 14 on the 6–20 Borg scale with 2 min of rest) either in hypoxia (FiO2 = 13.0%, HYP) or normoxia (NOR). A third trial replicating the HYP speed pattern was carried out in normoxia as a control (CON). Exercise responses were analysed comparing the average of 1st to 3rd exercise bouts to those of the 4th–6th, 7th–9th, 10th–12th and 13th–15th exercise bouts (block 1 versus 2, 3, 4 and 5).ResultsTreadmill speed was slower during block 4 (6.14 ± 0.67 versus 6.24 ± 0.73 km/h?1) and block 5 (6.12 ± 0.64 versus 6.25 ± 0.75 km/h?1) in HYP compared to NOR or CON (p = 0.009). Compared to NOR and CON, heart rate was +6–10% higher (p = 0.001), whilst arterial oxygen saturation (?12–13%) was lower (p < 0.001) in HYP. Perceived limb discomfort was lower in HYP and CON versus NOR (?21 ± 4% and ?34 ± 6%; p = 0.004).ConclusionsIn overweight-to-obese adults, perceptually regulated interval walks in hypoxia versus normoxia leads to progressively slower speeds along with lower limb discomfort and larger physiological stress than normoxia. Walking at the speed adopted in hypoxia produces similar psycho-physiological responses at the same absolute intensity in normoxia.  相似文献

目的探讨基于心肺运动试验(CPET)客观定量评估的个体化运动在高血压患者中的应用及对血压、血压变异性、脂代谢和脉搏波波形特征的影响。方法选取2019年1月至2019年6月济宁医学院教职工中的40例高血压患者,男23例,女17例,年龄(48.5±4.6)岁,年龄范围为40~58岁。采用随机数表法将患者随机分为常规组和CPET组,每组20例。常规组给予常规生活方式干预,CPET组在常规组的基础上给予基于CPET客观定量评估的个体化运动。观察两组患者干预前后静息血压、血清低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、总胆固醇、甘油三酯、24 h收缩压标准差(24 hSSD)、24 h舒张压标准差(24 hDSD)、日间收缩压标准差(dSSD)、日间舒张压标准差(dDSD)、夜间收缩压标准差(nSSD)、夜间舒张压标准差(nDSD)和桡动脉脉搏波切迹幅度。结果 CPET组干预后收缩压[(126.8±8.2)mmHg(1 mmHg=0.133 kPa)]和舒张压[(75.3±6.1)mmHg]水平均低于常规组[(132.1±6.2)mmHg、(79.7±6.4)mmHg],差异有统计学意义(P<0.05)。CPET组患者干预后LDL-C[(2.2±0.5)mmol/L]、总胆固醇[(4.8±0.6)mmol/L]、甘油三酯[(1.2±0.4)mmol/L]水平均显著低于常规组[(3.4±0.4)mmol/L、(5.3±0.7)mmol/L、(1.8±0.3)mmol/L],HDL-C[(1.5±0.2)mmol/L]高于常规组[(1.1±0.1)mmol/L],差异均有统计学意义(P<0.05)。CPET组干预后24 hSSD[(10.3±4.3)mmHg]、dSSD[(11.6±3.8)mmHg]、nSSD[(9.5±4.1)mmHg]、24 hDSD[(9.7±2.8)mmHg]、dDSD[(11.2±3.3)mmHg]、nDSD[(8.2±2.8)mmHg]水平低于常规组[(13.4±5.4)mmHg、(14.3±4.8)mmHg、(12.1±3.4)mmHg、(12.4±3.4)mmHg、(13.0±4.2)mmHg、(10.1±3.8)mmHg],差异有统计学意义(P<0.05)。CPET组干预后脉搏波切迹幅度高于常规组,差异有统计学意义(P<0.05)。结论基于CPET客观定量评估的个体化运动用于高血压干预可改善患者的血压控制效果,降低血压变异性,改善脂代谢,增加脉搏波切迹幅度,改善血管弹性水平,具有临床推广价值。  相似文献

Introduction:Exercise has been believed to have positive effects on blood glucose control in patients with type 2 diabetes mellitus. However, few medical evidences have been found to ascertain which type of exercise has the best effect on blood glucose control in diabetes and which type of exercise is more acceptable. The purpose of this study is to compare the effects and acceptability of different exercise modes on glycemic control in type 2 diabetes patients by using systematic review and network meta-analysis.Methods and analysis:Relevant randomized controlled trial studies will be searched from PubMed, EMbase, CochraneCENTRAL, CNKI, VIP, and Chinese medical paper libraries. Primary outcome indicators: glycosylated hemoglobin and dropout rate of the research (number of dropouts/numbers of initially enrolled subjects). Secondary outcome measures: fasting blood glucose, body weight, total cholesterol (TC), low-density lipoprotein cholesterol (LDL), high-density lipoprotein cholesterol, triglycerides (TG), diastolic pressure, systolic pressure (SBP). Two reviewers are arranged to screen Title, Abstract, and then review full text to further extract data. Standard meta-analysis and network meta-analysis of the data are performed afterward. Methodological quality assessment is planned to be conducted using Cochrane risk of bias tool. The outcome will be analyzed statistically according to Bayesian analysis methods. After that, subgroup analysis is conducted on the duration of intervention, whether there is supervision of intervention, frequency of intervention per week, age, gender, and medication use.Trial registration number:PROSPERO CRD42020175181Discussion:The systematic review and network meta-analysis include evidence of the impact of different exercise modes on blood glucose control in type 2 diabetes mellitus. There are 2 innovative points in this study. One is to conduct a classified study on exercise in as much detail as possible, and the other is to study the acceptability of different exercise modes. The network meta-analysis will reduce the uncertainty of intervention and enable clinicians, sports practitioners, and patients to choose more effective and suitable exercise methods.Ethics and dissemination:The findings of the study will be disseminated through publications in peer-reviewed journals and scientific conferences and symposia.
